I am going to be writing a proposal for the Texas Education Agency 21st Century Community Learning Center this winter and have written a few in the past.  While I don’t want your proposal to beat mine, here are a couple of tools that you will find useful in designing your program.

The National Center for Afterschool – AfterSchool Training Toolkit

The AfterSchool Alliance

The Finance Project’s Out-of-School Time Clearinghouse, which supports sustainability and is also available at The Out-of-School Time Clearinghouse

The Harvard Family Research Project (HFRP), which provides a bibliography on out of school time

National Afterschool Association and National Institute on Out-of-School Time, are sources for standards and professional development

Communities In Schools is the nation’s largest dropout prevention organization and a provider of services for 21st CCLC (disclaimer, I am a board member of CIS in Corpus Christi).
